NEED ASSISTANCE?

If you are the recipient of mental health, substance abuse services, or services related to a developmental disability or traumatic brain injury, or you are the caregiver/guardian of a “consumer” of one of these services, you may have some concerns or questions. Eastpointe is responsible for oversight of these services provided in Duplin, Lenoir, Sampson, and Wayne Counties. Our Agency includes a “Customer Services Department” that is here to assist you with any complaints, questions, or concerns.

You may contact Eastpointe's Customer Services Department by calling 1-800-513-4002 Monday through Friday between 8:00am and 5:00pm, or complete the Customer Services Assistance Form below. Note: You must click the "Send to Customer Services" button at the bottom of the form to complete the form.
